<abstract xmlns="http://www.w3.org/1999/xhtml"><sec id="j_1905-7415.0903.398_s_001_w2aab3b7b4b1b6b1aab1c16b1Aa"><h3>Background</h3><p><italic>Chlamydia trachomatis</italic>, <italic>Neisseria gonorrhoeae</italic>, and human papillomavirus (HPV) infections among women and girls may be symptomatic or asymptomatic.</p></sec><sec id="j_1905-7415.0903.398_s_002_w2aab3b7b4b1b6b1aab1c16b2Aa"><h3>Objectives</h3><p>To survey and identify <italic>C. trachomatis</italic>, <italic>N. gonorrhoeae</italic>, and HPV infections in symptomatic and asymptomatic women and girls in Bangkok and surrounding neighborhoods using molecular techniques, evaluate the use of doxycycline treatment for clinically symptomatic patients infected with <italic>C. trachomatis</italic>, and identify possible genetic mutations associated with persistence and/or drug resistance.</p></sec><sec id="j_1905-7415.0903.398_s_003_w2aab3b7b4b1b6b1aab1c16b3Aa"><h3>Method</h3><p>We enrolled 150 women and girls with inflamed urogenital tracts and 134 asymptomatic controls, both without a history of HIV infection or cervical cancer in this prospective study. Genomic DNAs were extracted, and <italic>C. trachomatis</italic>, <italic>N. gonorrhoeae</italic>, and HPV infections were detected using established PCR primers and protocols. PCR controls included no confirmatory template controls or human β-globin. Patients infected with <italic>C. trachomatis</italic> were treated with doxycycline and re-examined after treatment. <italic>C. trachomatis</italic> major outer membrane (<italic>ompA</italic>) and tryptophan synthesis A (<italic>trpA</italic>) genes were sequenced to identify possible genetic mutations associated with persistence and/or drug resistance.</p></sec><sec id="j_1905-7415.0903.398_s_004_w2aab3b7b4b1b6b1aab1c16b4Aa"><h3>Results</h3><p><italic>C. trachomatis</italic>, <italic>N. gonorrhoeae</italic>, and HPV were detected in 22%, 6%, and 48% of symptomatic, and in 3%, 16%, and 10% of asymptomatic women and girls, respectively. Coinfection with <italic>C. trachomatis</italic> and HPV were frequent in the 15-34 year age group, and associated with upper urogenital tract symptoms. Doxycycline was not considered effective for <italic>C. trachomatis</italic> infection. Several nonconserved amino acid changes were detected in <italic>C. trachomatis</italic> ompA and trpA.</p></sec><sec id="j_1905-7415.0903.398_s_005_w2aab3b7b4b1b6b1aab1c16b5Aa"><h3>Conclusion</h3><p>We found different distributions of these pathogens among symptomatic and asymptomatic patients. We also found doxycycline treatment failures, and mutated trpA supported persistent <italic>C. trachomatis</italic> infections.</p></sec></abstract>
