<abstract xmlns="http://www.w3.org/1999/xhtml"><p>Air may extend to the retroperitoneal space from retroperitoneal perforation of a hollow viscus, infection of the anterior pararenal space with gas-forming organisms and from pneumothorax or pneumomediastinum [<xref ref-type="bibr" rid="j_jccm-2016-0029_ref_001_w2aab2b8b3b1b7b1ab1ab1Aa">1</xref>]. Rare pathologies, such as open reduction and internal fixation of femoral fractures and anaerobic abscess of the hip joint have also been described in relation to this complication [<xref ref-type="bibr" rid="j_jccm-2016-0029_ref_001_w2aab2b8b3b1b7b1ab1ab1Aa">1</xref>,<xref ref-type="bibr" rid="j_jccm-2016-0029_ref_002_w2aab2b8b3b1b7b1ab1ab2Aa">2</xref>]. A rare case of pneumoretroperitoneum caused by insufflation of air during an attempt to achieve epidural anesthesia is described.</p></abstract>
