BACKGROUND. Starting from the European and American guidelines regarding sedation during gastrointestinal investigations and continuing with the American Society of Anesthesiologists (ASA) recommendations, we have developed a software that helps the ENT specialist to decide whether the patient can undergo a moderate sedation in the operating room, or in the outpatient office, during the drug-induced sleep endoscopy.
MATERIAL AND METHODS. The application is written in C#, using the. NET Framework, and is available for the Windows operating system. The application has a very simple and user-friendly interface, and a bare-bones implementation, focusing strictly on the decision-making process.
RESULTS. The application involves the information obtained from patient history, clinical examination and polysomnography. After filling all the patient’s data, it will show the result that can be one of the following: ”treatment can be ambulatory”, “treatment should be done in the hospital” or “treatment is not recommended”.
CONCLUSION. The decision-making software application may substitute the evaluation of the anaesthesiologist during drug-induced sleep endoscopy in certain conditions.
Caplan’s syndrome, known as rheumatoid pneumoconiosis, was first described by Anthony Caplan in 1953, who identified a rare lung disorder found in coal mine workers with rheumatoid arthritis. Although Caplan’s syndrome was found in patients with a variety of pneumoconioses, it mostly affects individuals with long exposure to crystalline silica. We present a case of Caplan’s syndrome in a patient with advanced stage of rheumatoid arthritis and silicosis.
Entrapment syndromes of the upper limb are common neuro-muscular-skeletal pathology in musician instrumentists. From this group of morbid entities, the most prevalent worldwide is carpal tunnel syndrome closely followed by the cubital tunnel syndrome and de Quervain stenosing tenosynovitis. Due to their distinctive etiopathogenic correlation with exposure to specific occupational factors linked to instrument interpretation and professional environment, these diseases raise a medical challenge and constitute a socioeconomic and professional burden with legal branchings and implications for individuals and society. These syndromes develop isolated or more often in various associations with each other in a clinical pattern that has been described under the model of “double crush” syndrome by Upton and McComas. From its inception in 1973 until the present time, this clinical model has been a point of interesting debate between various specialists worldwide. This model underlines an already lesioned neuron’s susceptibility and vulnerability for further neural damage at a different level from the initial lesion. The sophisticated clinical presentation of this “double or multiple crush” syndrome is due not only to overlapping symptomatology from each contributing neuro-muscular-skeletal pathology or lesional site but also to other local or systemic conditions such as trauma, diabetes, osteoarthritis, thyroid disease, obesity, etc. The occupational factors such as repetitive movements, strain and overload, vibrations, ergonomics, and others all contribute to the creation and progression of the morbid process. We cannot overstate the implications of understanding these complex relations and interdependencies between the factors mentioned above as they are essential not only for the diagnosis of these neuropathies but also for the treatment, rehabilitation, and occupational reinsertion of the patients. The studies support the fact that both lesional sites need to be medically addressed for an optimal outcome and resolution. We present the case of a female violinist with bilateral multiple neuro-muscular-skeletal pathologies of the upper limb treated previously invasively and conservatively over several years by various specialists without a satisfactory clinical resolution of the symptomatology or any professional and legal measures taken.
In the perspective of the General Theory of Fighting Arts, an analysis of socio-cultural factors that determine the opposition of the role of a teacher of martial arts (Jap. sensei) to the role of a sports trainers was undertaken. The structural cultural context, cultural patterns, and social institutions resulting from divergent goals were taken into account. The roles of teachers and trainers result from these conditions. The existence of the separate roles of the master-teacher in martial arts and the sports trainer was established. These roles manifest themselves in different relations with students or players. Democratic and egalitarian interactions in sports teams include player and coach relations. In traditional martial arts, the dominance of the teacher is more accepted. However, there is also a social position combining the features of the sensei and the trainer that is typical for combat sports that are also martial arts (participating in sports competitions). As there are relationships of subordination in the hierarchical societies of Japan and Korea, there is no problem with recognizing the primary role of the sensei in these cultures. The position of the master-teacher is also sanctified by tradition. Reducing educational systems, which are the paths of martial arts, to oriental varieties of sports would be a serious factual mistake.
The definition of COVID-19 as occupational disease follows the investigation of any other occupational disease caused by an infectious agent. The risk is not equal for all occupations and the occupational physician has to assess the working conditions to conclude a diagnosis of occupational COVID-19. In the pandemic context, employees face also other occupational hazards. The high level of work load and the scarce resources lead to stress, physical and mental exhaustion and irregular sleep. The protection measures, of undisputable benefit, increase the risk for contact dermatitis. There is a high probability for medium and possible long term effects of COVID-19, such as the post-traumatic stress disorder or the respiratory sequelae. These consequences need to be acknowledged and properly manged by the medical team taking care of the patient. This review presents the main characteristics of the occupational related disorders during and after the current pandemia.
Food and nutritional (in)security remain an important matter of concern, especially in developing countries. Despite the efforts to enhance food security among smallholder soybean households, the proportion of the undernourished population in Butere Sub-County still remains high for unknown reasons. Therefore, the purpose of this paper is to examine the determinants of food security among smallholder soybean households in Butere Sub-County, Kenya. The study adopted the exploratory research design. A multi-stage sampling procedure was used to select a sample of 201 respondents. Cross-sectional data were gathered through face-to-face interviews using pretested semi-structured questionnaires and analyzed using ordered logistic regression model. Household Food Insecurity Access Scale was used to measure and categorize the soybean household food (in)security status. The results revealed that the household food (in)security status differed across soybean households and was greatly influenced by an interplay of socio-economic, market, and institutional factors. Age of the household head negatively influenced food security, whereas the level of soybean commercialization, education, livestock units, network density, extension visits, and credit access were positively associated with household food security. The study recommends policy interventions that seek to ensure intensive literacy development, frequent extension and training, improved access to credit, and reinvestment in productive assets or inputs for increased production, commercialization and food security. Strengthening of social ties and increased allocation to safety net programs for the aged, vulnerable, and resource-poor households are also recommended.
Occupational asthma, the most common occupational respiratory disease in industrialized societies, accounts for 5-10% of all cases of asthma diagnosed in the world. The number of cases is increasing given the development of the “consumer society”. We aim to discuss a case of occupational asthma that we have confirmed using internationally validated methods.
The aim of the study was to investigate the effect of an interdisciplinary program of Greek folk dance, with topics from history and geography on Middle School students’ goal orientation and anxiety level. The sample consisted of 260 students (134 boys & 126 girls). The experimental group (n=144) followed the interdisciplinary four-week program (two lessons per week) while the control group (n=126) followed the corresponding typical physical education program. For the data collection, the questionnaire used was the “Goal orientation” by Papaioannou, Milosis, Kosmidou, and Tsiggilis (2002) and the Greek version (Kakkos & Zervas, 1996) of “Competitive State Anxiety Inventory-2” (Martens et al., 1990). The students filled in the questionnaires before and after their participating in each program. Descriptive statistics, reliability analysis, and Repeated Measures ANOVA were used. Results showed that a. Cronbach’s alpha was satisfactory. b. Τhe experimental group decreased significantly the levels of “ego-strengthening” and “ego-protection”, and on the contrary significantly increase the “personal development”, and “social acceptance”. c. Experimental group’s boys and girls decreased their “ego-strengthening” and “ego-protection”, and enhanced their “personal development” and “social acceptance” more than their control group peers. d. The experimental group decreased the levels of somatic and cognitive anxieties and increased significantly their self-confidence. e. male students increased their self-confidence more than female students. These findings support the view that an interdisciplinary program of traditional Greek dance with issues from history and geography enable us to reduce the rates of ego-strengthening, ego-protection, somatic and cognitive anxiety while simultaneously increases students’ personal development, social acceptance, and self-confidence
Boesenbergia rotunda or Temu kunci is a herb belonging to the Zingiberaceae family and wildly cultivated in Malaysia by rhizome. Temu kunci rhizome is commonly used in traditional medicines to cure stomach aches, promote appetite and gout. Due to its potential to be developed as one of Malaysia’s herbal products, information on their agronomic requirements is needed. This study was conducted to investigate the effects of the different combination of the growing medium on B. rotunda growth and yield. Topsoil, peat moss, sand and chicken manure with four different ratios have been used as a planting medium. The potted plant was arranged in a randomised, complete block design with five replicates. The growth parameter was measured during harvesting time. The results showed that there was no significant difference in plant height, number of leaves, tiller number, fresh and dry shoot weight and fresh and dry root and rhizome weight in all treatments. It can be argued that this is because B. rotunda can be grown in different kinds of planting medium. Based on this study, it was suggested that topsoil be used for Temu Kunci planting, since it is easily obtained and requires less money.
