Introduction Severe hypercholesterolaemia with a total cholesterol (TC) concentration above 25 mmol/L (∼1000 mg/dL) is an extremely rare condition. The most widely recognised cause is homozygous familial hypercholesterolaemia (HoFH) presenting with an increased level of LDL-cholesterol (LDL-C) and accelerated advanced cardiovascular disease (CVD) ( 1 ).
