Introduction It is recognised that, relative to the population of small- and medium-sized enterprises (SMEs), high-growth SMEs (HGSMEs) make a disproportionate contribution to economic growth. HGSMEs are frequently cited by policymakers and idealised by entrepreneurs with growth ambitions ( Volery et al., 2015 ). In 1966, Laron et al. first described growth hormone insensitivity (GHI) in 3 children from a consanguineous Yemenite family who had extreme growth failure and a phenotype of hypopituitarism, but high concentrations of circulating GH [ 1 ].
