Comparison of a Web-Based Dietary Assessment Tool with Software for the Evaluation of Dietary Records / Primerjava Spletne Aplikacije in Računalniškega Programa Za Ovrednotenje Prehranskih Dnevnikov

Abstract

Background. Dietary assessment in clinical practice is performed by means of computer support, either in the form of a web-based tool or software. The aim of the paper is to present the results of the comparison of a Slovenian web-based tool with German software for the evaluation of four-day weighted paper-andpencil- based dietary records (paper-DRs) in pregnant women.

Methods. A volunteer group of pregnant women (n=63) completed paper-DRs. These records were entered by an experienced research dietitian into a web-based application (Open Platform for Clinical Nutrition, OPEN, http://opkp.si/en, Ljubljana, Slovenia) and software application (Prodi 5.7 Expert plus, Nutri- Science, Stuttgart, Germany, 2011). The results for calculated energy intake, as well as 45 macro- and micronutrient intakes, were statistically compared by using the non-parametric Spearman’s rank correlation coefficient. The cut-off for Spearman’s rho was set at >0.600.

Results. 12 nutritional parameters (energy, carbohydrates, fat, protein, water, potassium, calcium, phosphorus, dietary fiber, vitamin C, folic acid, and stearic acid) were in high correlation (>0.800), 18 in moderate (0.600-0.799), 11 in weak correlation (0.400-0.599), while 5 (arachidonic acid, niacin, alphalinolenic acid, fluoride, total sugars) did not show any statistical correlation.

Conclusion. Comparison of the results of the evaluation of dietary records using a web-based dietary assessment tool with those using software shows that there is a high correlation for energy and macronutrient content.

Quality indicators and accreditation as a tool for quality management in healthcare in Slovenia

Kazalniki kakovosti in akreditacija kot orodja za vodenje kakovosti v zdravstvu v Sloveniji

Uvod. Proučevanje odstopanj od pričakovanih rezultatov dejavnosti je osnovno orodje za izboljšanje kakovosti. Tudi v zdravstvu se tovrstno proučevanje odvija, zlasti z določitvijo kazalnikov kakovosti in postavitvijo standardov ter presojo njihovega spoštovanja skozi sistem akreditacije. V Sloveniji so se, še posebej v zadnjem letu, vrstile dejavnosti, namenjene razvoju tega področja.

Metode. Potrebno se je bilo uskladiti in se dogovoriti za nacionalni nabor kazalnikov kakovosti ter pogoje za vzpostavitev ustreznega modela akreditiranje zdravstvenih ustanov v Sloveniji. Dogovor zahteva usklajevanje vseh ključnih akterjev in bistveno prispeva k razjasnitvi odnosov odgovornosti med partnerji v sistemu zdravstvenega varstva.

Rezultati. Oblikovan je bil razširjen nabor 72 kazalnikov kakovosti, ki se spremljajo na nacionalni ravni. Postavljene so bile tudi usmeritve glede akreditiranje, ki predvidevajo spodbude za akreditiranje pri mednarodno priznanih organizacijah. Pri tem je ključno upoštevanje nacionalnih in mednarodnih usmeritev, še posebej pa Direktive o pravicah pacientov do čezmejnega zdravstvenega varstva.

Zaključek. Skupna točka vseh partnerjev je zagotavljanje visoko kakovostne in varne zdravstvene obravnave za bolnika.

Internal auditing as a tool for improving efficiency in public health

Notranja revizija kot orodje za izboljšanje učinkovitosti poslovodenja v javnem zdravstvu

Notranjerevizijska dejavnost je dejavnost sodobnega časa. V Sloveniji v obdobju zadnjih desetih let zaznavamo izredno hiter razvoj in uveljavitev notranjega nadzora javnih financ na področju organizacij javnega zdravstva. Za proračunske uporabnike je notranjerevizijska dejavnost tudi predpisana z zakonom. Kot taka postaja v zadnjem času vse bolj vpeta tudi v poslovanje javnih zdravstvenih zavodov. Njeno delovanje označuje preizkušanje in ocenjevanje zadovoljivosti delovanja sistema notranjih kontrol in svetovanje pri njegovem izboljševanju zato, da bi zagotovili učinkovito in uspešno obvladovanje poslovnih tveganj bolnišnice. Poslovno tveganje razumemo kot vsak neugoden dogodek, ki vpliva na učinkovito in uspešno doseganje zastavljenih ciljev javnega zdravstvenega zavoda. Za obvladovanje tveganj je sicer odgovorno poslovodstvo, notranja revizija pa mu pri tem nudi ustrezno podporo. Svoje poslanstvo lahko notranja revizija izpolni uspešno le pod pogojem, da ima poslovodstvo ustrezno védenje in znanje o notranji reviziji.
