Integration of personal health systems into intelligent living environments

Integracija osebnih zdravstvenih sistemov v inteligentna bivalna okolja

Rešitve s področja informacijskih in komunikacijskih tehnologij (IKT) lahko prispevajo k boljši oskrbi kroničnih bolnikov in starejših. Posebej so aktualni osebni zdravstveni sistemi (angl. personal health systems - PHS), ki omogočajo oddaljeno spremljanje zdravstvenega stanja bolnikov, predvsem kroničnih. Drugi sklop aktualnih rešitev so inteligentna bivalna okolja, ki lahko bolnim in invalidnim osebam olajšajo vsakodnevna opravila in s tem podaljašajo obdobje samostojnega življenja doma.

Razširjenost kroničnih bolezni in tržni potencial, ki ga izkazujejo tako sistemi PHS kot rešitve inteligentnih domov, navajata na sklep, da obstaja krog potencialnih uporabnikov obeh sklopov storitev. Predvidevamo, da bi imela tesnejša integracija obeh vrst sistemov pozitivne učinke tako v smislu tehnične izvedbe in cen storitev kot tudi večje uporabnosti zaradi manjšega števila različnih uporabniških naprav oz. vmesnikov.

Koncept integracije storitev PHS v sisteme inteligentnih domov smo uresničili v okviru treh pilotnih projektov. Za Dom IRIS smo razvili sistem za spremljanje krvnega tlaka in ga integrirali v obstoječo rešitev za upravljanje doma. Delo smo nadaljevali v okviru projekta Inteligentni dom Telekom (IDT), pri katerem smo razvili celovitorešitev inteligentnega doma, namenjeno izvajanju storitev v operaterskem okolju. Tretji pilot je rešitev Moje zdravje, pri kateri smo zdravstvene storitve integrirali v TV vmesnik nove generacije.

Rezultati pilotnih projektov kažejo na smiselnost koncepta integracije sistemov tako s stališča učinkovitejše tehnične izvedbe kot tudi večje uporabnosti in nižje cene storitev. Nadaljnji izzivi so povezani z oblikovanjem poslovnih modelov, udejanjanjem navezave na zdravstveni informacijski sistem ter ustrezno vključitvijo zdravstvenih delavcev v storitve telemedicine.

The local help network to the homeless: case study of the city of koper, Slovenia

Abstract

Aim: To present the work of professionals and volunteers of the local help network that revolves around trying to help the homeless and to stimulate readers to critically assess the possible methods aimed towards the successful integration of those people into society.

Methods: In the city of Koper, we visited five governmental (GOs) and non-governmental organisations (NGOs): Red Cross Koper, Daybreak Association, Center for Social Work Koper, Diocesan Caritas Koper and Koper Prison; and interviewed 3-10 staff members at each organisation.

Results: For each organisation, we described its duties and activities, including its interconnection with other organisations, methods of integrating the homeless into the society and the personal thoughts of its staff members.

Conclusions: Both GOs and NGOs are necessary for providing effective assistance to people in need. NGOs excel at quickly responding to immediate needs. Their programs are usually implemented only as short-term resolutions. GOs on the other hand require a longer time to implement their concepts. Nonetheless, in contrast to NGO projects, they provide long-term stability. Even though people from remote parts of the society usually cooperate, the efforts of those who work with them are nothing short of exerting. They strive to achieve a general social acceptance of their ward population, which is the one thing those people need the most. Since only the society is truly capable of offering them a firm stepping stone towards escaping from the vicious circle in which they stray.

Older people and their strategies for coping with health risks

Starejši Ljudje in Njihove Strategije Shajanja S Tveganji Za Zdravje

Izhodišča: Članek obravnava problematiko ene najranljivejših skupin v družbi, tj. starejših, in sicer tveganja za zdravje, s katerimi se v vsakdanjem življenju srečujejo starejši po njihovi subjektivni percepciji. Nato na osnovi strategij shajanja analizira načine, na katere si starejši pomagajo, ko so soočeni z različnimi tveganji.

Metode: Prispevek temelji na raziskavi z dvostopenjskim modelom z integracijo kvantitativnih in kvalitativnih metod. Kvantitativni podatki ankete na kvotnem vzorcu (N = 558) so nadgrajeni v temeljnem kvalitativnem delu, ki s pristopom »grounded theory« z večstopenjskim kodiranjem analizira 35 polstrukturiranih poglobljenih intervjujev.

Rezultati: V percepciji starejših so tveganja za zdravje v veliki meri prevladujoča. Zdravstvene težave lahko močno ogrozijo kakovost življenja, ki jo kot najpomembnejšo kategorijo predstavlja neodvisnost. Starejši z različnimi aktivnimi in s pasivnimi strategijami, ki segajo od uporabe tehnologij do omejevanja in prejemanja socialne opore, shajajo z zdravje ogrožajočimi okoliščinami vsakdanjega življenja.

Razprava: Analiza strategij shajanja predstavlja primeren način raziskovanja, ki pokaže starejše kot dejavne akterje, ki sami skrbijo za lastno blagostanje. Na osnovi individualnih strategij lahko tudi posredno razberemo, na katerih področjih bi družba lahko storila več za avtonomnost in vključenost starejših prebivalcev naše družbe.

Razvoj sistema skupine primerljivih primerov v zdravstvu

Članek podaja vsebinski in časovni pregled razvoja sistemov plačevanja po skupinah primerljivih primerov. Sistem je bil sprva uveden kot managersko orodje za znižanje stroškov in povečanje učinkovitosti. Sistem plačevanja skupin primerljivih primerov je sledil razvoju medicine in sodeloval v izgradnji sistema kakovosti in varnosti bolnikov. V zadnjih letih so v nekaterih državah že pričeli poplačevati kakovost zdravstvenih izidov. Sodobni trend celostne obravnave bolnikov bo zahteval ponovno prilagoditev sistema plačevanja v zdravstvu.

The “ECG modifications induced by the disturbance of the circadian rhythm in night-shift workers (ECGNoct)” study protocol

Abstract

In many large cohort studies, the night shift constitutes a risk factor for developing cardiovascular disease and diabetes in workers. Current screening tests for people working in night shift include fasting glycaemia and electrocardiography. In fact, there are few studies focused on the description of the electrocardiographic changes after the night shift. This article describes the protocol of the “ECG modifications induced by the disturbance of the circadian rhythm in night-shift workers (ECGNoct)” study, which was initiated by the National Institute for Infectious Diseases “Prof. Dr. Matei Balș”. Nurses represent the target population.

The protocol includes a full medical and occupational history, lifestyle habits (smoking, alcohol, nutrition), anthropometric and blood pressure measurements, blood tests (fasting glycemia, total cholesterol, triglycerides and high density lipoprotein cholesterol) and electrocardiogram recording. For nurses working in (night) shifts, we will record the electrocardiogram before and soon after the night shift. A cross sectional study will analyze the incidence of the metabolic syndrome criteria, the cardio-metabolic diseases and the electrocardiographic modifications and will compare the results between the group of nurses working and the group of nurse who do not. Based on these results, a longitudinal study will test the hypothesis that night shift increases the risk for cardio-metabolic diseases and that the electrocardiographic modifications precede the clinical symptoms. The results of the study will provide data on the association of night shifts and other non-occupational risk factors with the cardio-metabolic diseases in this specific population of healthcare workers that potentially will integrate into the occupational medicine policies.
