Varices manifest as a major etiology of upper gastrointestinal bleeding in patients with chronic liver diseases, such as liver cirrhosis and hepatocellular carcinoma. By contrast, non-variceal upper gastrointestinal bleeding is rare. Pharmacological treatment differs between patients with variceal and non-variceal bleeding. Vasoconstrictors are recommended for the treatment of variceal bleeding, rather than non-variceal bleeding. In contrast, pump proton inhibitors are recommended for the treatment of non-variceal bleeding, rather than variceal bleeding. Herein, we present a case with liver cirrhosis and acute upper gastrointestinal bleeding who had a high risk of rebleeding (i.e., Child–Pugh class C, hepatocellular carcinoma, portal vein thrombosis, low albumin, and high international normalized ratio and D-dimer). As the source of bleeding was obscure, only terlipressin without pump proton inhibitors was initially administered. Acute bleeding episode was effectively controlled. After that, an elective endoscopic examination confirmed that the source of bleeding was attributed to peptic ulcer, rather than varices. Based on this preliminary case report, we further discussed the potential role of vasoconstrictors in a patient with cirrhosis with acute non-variceal upper gastrointestinal bleeding.

Acute-on-chronic liver failure (ACLF) is a potentially lethal syndrome, which is characterized by an acute deterioration of liver function in patients with chronic liver diseases. The present paper reported that an alcoholic cirrhotic patient with ACLF developed septic shock, hydrothorax, ascites, hepatic encephalopathy, acute kidney injury, and acute upper gastrointestinal bleeding at the same hospitalization and was successfully rescued by pharmacotherapy alone without any invasive intervention.

Background. Carbon ions (12C6+) are high linear energy transfer (LET) radiation characterized by higher relative biological effectiveness than low LET radiation. The assessment of tumour radiosensitivity would be particularly useful in optimizing the radiation dose during radiotherapy. The aim of the current study was to evaluate the potential value of the neutral comet assay and γH2AX foci assay in assessing 12C6+ radiosensitivity of tumour cells.
Materials and methods. The doses of 12C6+ and X-rays used in the present study were 2 and 4 Gy. The survival fraction, DNA double-strand breaks (DSB) and repair kinetics of DSB were assayed with clonogenic survival, neutral comet assay and γH2AX foci assay in human cervical carcinoma HeLa cells, hepatoma HepG2 cells, and mucoepidermoid carcinoma MEC-1 cells at the time points of 0.5, 4, 16 and 24 h after 12C6+ and X-rays irradiation.
Results. The survival fraction for 12C6+ irradiation was much more inhibited than for X-rays (p < 0.05) in all three tumour cell lines tested. Substantial amounts of residual damage, assessed by the neutral comet assay, were present after irradiation (p < 0.05). The highest residual damage was observed at 0.5 or 4 h, both for 12C6+ and X-ray irradiation. However, the residual damage in HeLa and MEC-1 cells was higher for 12C6+ than X-rays (p < 0.05). The strongest induction of γH2AX foci was observed after 30 min, for all three tumour cell lines (p < 0.01). The franction of γH2AX foci persisted for at least 24 h after 12C6+ irradiation; in HeLa cells and MEC-1 was higher than after X-ray irradiation (p < 0.05). The correlation coefficients between the clonogenic survival, neutral comet assay and γH2AX foci assay were not statistically significant, except for some tumour cells at individual irradiation doses and types.
Conclusions. Our study demonstrated that the neutral comet assay and γ-H2AX foci assay could be used to assess the radiosensitivity of 12C6+ in human tumour cells.

Effect of Stubble Heights and Treatment Duration Time on the Performance of Water Dropwort Floating Treatment Wetlands (FTWS)
Floating treatment wetlands (FTWs) with Water Dropwort (Oenanthe javanica) were established in winter to investigate their potential role in the purification of eutrophicated water, and to identify the effects of different stubble heights of the Water Dropwort on the performance of the FTWs. The results of the experiments demonstrated: The Water Dropwort FTWs were effective in buffering the pH of the experimental water. The Water Dropwort FTWs were efficient in purifying eutrophicated water, with removal rate for total nitrogen (TN), total phosphorus (TP), ammonium nitrogen (NH4 + - N), and nitrate nitrogen (NO3 - - N) at 91.3, 58.0, 94.6, and 95.5% in the 15-day experiment, respectively. No significant difference in the purification effect was found among different stubble heights of Water Dropwort FTWs. Significant differences between the zero control and the FTWs were found for the removal of TP in the first 11 days; and for the removal of NH4 + - N in the first 4 days. No significant difference was found between the zero control and the FTWs for NO3 - - N in the first 4 days, but significant difference was detected after day 4. The optimum treatment duration time for the FTWs with Water Dropwort will depend on the nutrients to be removed. These results will provide basis for further application of the FTWs at large scale, as well as for future studies on the mechanism of nutrient removal process.

Background: The objective of this study is to detect HMGA2 expression in renal carcinoma to explore its relationship with clinicopathology and its significance in prognosis. Expressions of HMGA2 mRNA and protein were detected in 50 renal carcinoma specimens, 50 corresponding adjacent normal kidney tissue samples and 40 renal benign tumour specimens via reverse transcription polymerase chain reaction and immunohistochemical assay.
Methods: Expression analysis was performed along with clinical data analysis. The relative expression levels of HMGA2 mRNA in renal carcinoma, renal benign tumour tissues and adjacent normal renal tissues were 0.84±0.23, 0.19±0.06 and 0.08±0.04, respectively. HMGA2 protein positive rates were 68.0%, 7.5% and 2.0%, with a significant difference (P<0.05). HMGA2 expression was not significantly correlated with gender, age, tumour size and histological type (P>0.05), but was significantly correlated with TNM stages and lymph node metastasis (P<0.05).
Conclusions: The expressions of HMGA2 gene and protein in renal carcinoma were closely correlated with tumour formation, progression and metastasis. HMGA2 may become a powerful new pathological marker and prognostic factor for renal carcinoma.

Bladder cancer (BCa) is one of the most common urinary cancers. The present study aims to investigate whether Paeoniflorin (Pae) can exert inhibitory effects on BCa. The results showed that Pae inhibited proliferation of human BCa cell lines in a concentration- and time-dependent manner. Pae and cisplatin (Cis) synergistically inhibited the growth of tumours in RT4-bearing mice. Pae treatment neutralized the body loss induced by Cis. Moreover, Pae induced apoptosis in RT4 cells and increased the activities of caspase3, caspase8 and caspase9. Western blotting and immunohistochemical analysis revealed that the phosphorylated signal transducer and activator of transcription-3 (p-STAT3) level were decreased in Pae-treated RT4 cells and Pae-treated tumour-bearing mice. Furthermore, STAT3 transcriptional target B-cell lymphoma-2 was decreased in Pae-treated RT4 cells. Interestingly, Pae prevented translocation of STAT3 to the nucleus in RT4 cells. Collectively, Pae inhibits the growth of BCa, at least in part, via a STAT3 pathway.

Objective To investigate the effects of individualised treatment with peginterferon alpha-2a (40 kD) plus ribavirin in Chinese patients with CHC.
Methods Total of 297 consecutive Chinese patients were enrolled, including 250 naïve cases and 47 cases who were previously treated. Treatment duration was determined according to viral genotypes, prior treatment history and viral responses at week 4, 12 and 24.
Results Totally, 235 patients (79.1%) completed treatment and 186 (87.3%) achieved SVR. And 219 out of 289 (75.8%) patients achieved HCV RNA negative at week 4 (RVR) and 259 of 276 (93.8%) at week 12. Among the 164 patients with RVR who completed follow-up, 158 (96.3%) achieved SVR. Patients with RVR had lower baseline viral loads than patients without RVR (P = 0.034). The positive predictive value (PPV) of RVR for SVR was 90.7% (OR 2.10 vs. non-RVR, 95% CI: 0.50 - 8.7). Similar outcomes were observed among patients with HCV undetectable at week 12.
Conclusions Complete viral suppression by week 4 is associated with a high rate of treatment success in treatment naïve and experienced patients receiving individualized CHC therapy.
