Communication as a Key Issue in the Care of Diabetes Mellitus

Abstract

The physician-patient communication has an essential role in establishing and supporting the relationship between these two partners. Moreover, modern medicine highlights the patient-centered approach. Publications assessing the impact of an efficient physicianpatient communication on medical care results in diseases such as diabetes and hypertension have revealed a positive correlation between patient’s satisfaction about the communication with the physician and values of blood pressure, glycated hemoglobin and pain intensity. Interventions needed in both doctors and patients for developing communication abilities were paid special attention in order to achieve an appropriate improvement in their communicative interaction during periodical appointments. In the field of diabetes mellitus, the medical challenge is to improve patients’ knowledge about medical care; this aim is achieved only by therapeutic education, using high-quality communication techniques.

Data Regarding the Prevalence and Incidence of Diabetes Mellitus and Prediabetes

Abstract

Diabetes Mellitus (DM) represents one of the highest challenges in our century, due to the fact that in the last 20 years the number of patients with DM has doubled, at present affecting hundreds of millions of people worldwide, both in developed countries and in developing ones, as well. One of the most serious consequences of this increase is the onset of type 2 DM in children, adolescents and young people, the main causes being an unhealthy lifestyle: unhealthy food, lack of physical exercise, which, most of the times, lead to obesity. Also, DM is often associated to micro and macrovascular complications, thus determining disabilities and high costs in the healthcare systems, respectively. DM is one of the main causes of death all over the world, a reason for which there are required prevention programs worldwide.

The Relationship between Hepatic Steatosis, inflammation and insulin Resistance in type 2 Diabetes with Metabolic Imbalance

Abstract

Aim. Non-alcoholic fatty liver disease (NAFLD) and type 2 diabetes mellitus (T2DM) are in a bidirectional relationship. This prospective study focused on associations between parameters common to the pathogenesis of insulin resistance, inflammation and hepatic steatosis in T2DM patients with metabolic imbalance.

Methods. We used clinical data, insulin resistance and inflammation indices, and hepatic steatosis markers from 120 patients.

Results. The patients (44% men, mean age 58) had a mean body mass index (BMI) of 32 kg/m2 and mean T2DM history of 6 years. With exceptions, significant correlations were found between metabolic, inflammatory and hepatic parameters.

Conclusions. In T2DM patients with poor glycemic control, hepatic steatosis correlates significantly with insulin resistance and inflammation. Increased prevalence and poor prognosis of these diseases together justify the need for NAFLD screening of diabetic patients.
